Note to 6th Graders:

Mrs. Mena will send home a packet of info in January for your required 7th grade physical exam.  You need a record of it for entry into your new school. It is very important to get it done before school ends. If you do, you can submit it to Mrs. Mena or to your new school. Any questions, call the office. Mahalo.  

The Kailua Elementary Health Aide, Mrs. Mena is available to assist your child in case of illness or injury in school.  If your child is injured or becomes ill at school, she will provide emergency first aid and will notify you by phone.   When called, please make every effort to pick up your child promptly in the health room located in the front office.

ADMINISTERING OF MEDICATION BY SCHOOL PERSONNEL

Medication may be administered in school only after the proper forms have been completed by the parent, physician, and the school nurse (Form 36).  No medication is given to students without the properly completed forms.  School personnel shall not be responsible for reminding children to take their medication.

 

COMMUNICABLE DISEASE REFERENCE

The Public Health Regulations Chapter V, recommends for the control of communicable diseases that children with skin diseases, scabies, chickenpox, pink eye, conjunctivitis, influenza, measles, mumps, rubella, scarlet fever, strep throat and other strep infection or active tuberculosis be excluded from school.  A doctor's certificate will be required that the illness is/was under treatment.

 

DISEASE EXCLUSION FROM SCHOOL

Children with fever, headaches, sore throat, cough or "cold", noticeable rash, diarrhea or vomiting, or other obvious signs of illness should be excluded from school immediately to obtain medical care and diagnosed by a physician who will determine what measures to take and whether the illness is reportable to the Health Department.  (IF YOUR CHILD HAS ANY OF THE ABOVE, PLEASE DO NOT SEND HIM/HER TO SCHOOL.)

Hawai'i Keiki Nurse

Did you know that your school has a Hawaii Keiki Nurse Practitioner? What does that mean for you and your keiki? It means that with your consent your child can receive expanded health services such as getting Tylenol or Ibuprofen for an illness, having a nebulizer treatment to control an asthma exacerbation and receive treatment and prescriptions for common health issues when unable to see your primary healthcare provider. Hawaii Keiki is a program that is here in your school aiming to reduce absences due to illness and help keep your child healthy and ready to learn!  If you would like your child to be eligible for services please sign and return the Hawaii Keiki Consent at your earliest convenience.

Hawai'i Keiki has made fillable consent forms available.  Save the pdf file to your desktop, fill the form in with your information and either email or print it.  Both fillable consent forms need to be filled in and turned into the office.
